About transitional care

UF Health Transitional Care provides medically-supervised therapy by physical, occupational and speech therapists in a comfortable, friendly setting. This separately licensed 22-private-bed facility provides the nursing and rehabilitative support necessary to help you regain independence and return home with confidence. Patients at UF Health Transitional Care typically receive one to two hours of therapy daily and stay less than two weeks, depending on progress and discharge destination. Each patient receives an individually-designed care plan with input from a physician, involved nurses and therapists, and case manager.
